The Respite Care Project is funded by the Department of Ageing, Disability and Home Care
(DADHC) under the Home and Community Care Program (HACC) to assist and support the
existing ethno-specific Day Cares in the Illawarra.
In addition, the project aims to identify the needs of the target group -- from linguistically
and culturally diverse backgrounds -- and develop strat egies to meet those needs.

Who can you refer to the service?
Referrals to this service can be made by other service provides such as the Aged Care Assessment team (ACAT), Community Health Doctors, carers and friends, or even yourself.
The eligibility criteria:
The Coordinator will accept a referral on the basis of providing:
- Respite to carers
- Support to frail older people, and younger people with a disability and their carers,
particularly those from a culturally and linguistically diverse background.
The number of referrals accepted depends on the number of clients already utilising the service and the resources available to the service. Where appropriate, we will refer clients to other relevant services and/or your name will be put on an eligibility list.
Client Assessment:
Clients will be assessed in accordance with the Client Information and Referral Record (CIARR) in addition to specific information requested by individual services to identify your needs. |
